1. Letter of intent addressed to:

Indicate the following in the letter:

a. School, school year and levels attended by the applicant

b. Contact numbers

*may be filed as early as the 2nd semester effective November 10, 2010 at the Registrar's Office

 

2. Photocopy of the Birth Certificate

3. Photocopy of the current report card with the 2nd quarter grades and previous report cards.

4. Photocopy of ACR/ICard and Passport/Visa (if Alien)

 

*Schedule of evaluation usually starts on the 1st week of February the following year
