II. APPLICATION REQUIREMENTS:

A. AGE

Minimum age of 4 years and 4 months and a maximum age of 5 years and 10 months by June 1, 2012. Those born on August 1, 2006 to February 29, 2008 are eligible.

B. DOCUMENTS

1. 2 recent 1" x 1" colored pictures with the name of the applicant written at the back of the photos.

2. Xerox copy of Birth Certificate with Registry Number (Bring the original copy for verification purposes)

3. Xerox copy of Baptismal Certificate (if baptized Catholic)

4. Xerox copy of ACR/ICard and passport/visa (if alien). (Bring the original copies for verification purposes)

5. Accomplished ICA Student Health Record Form (to be submitted together with the application form). The FORM will be released upon submission of the other documents.

6. 2011-2012 Certificate of Enrollment in a Nursery School.
